For your convenience, you may print the following history form and complete it prior to your appointment visit to facilitate prompt service when you arrive at our office. If you have any questions, please feel free to contact us.
On
the day of your appointment, please bring in all of the eyeglasses and
sunglasses you are currently using. If
you wear contact lenses, please wear the contact lenses on your normal
wearing schedule. Continue to
use all prescribed eye medications as you have been directed. Either know or bring a list of all medications your are
taking on a regular basis.
